How to report a mistake or suggest a guide
1Tell us the page address and the sentence or figure you think is wrong.
2Include a link to the official source (a PhilHealth circular, advisory or page) if you have one.
3Tell us what changed and, if you know it, the date of the change.
4We review the report, update the guide and show a new last-reviewed date. We may write back if we need more detail.
We do not receive PhilHealth PINs, passwords or ID scans through this form, and we cannot change your PhilHealth record. To correct your record, use the steps in the PhilHealth requirements guide or visit your Local Health Insurance Office.
