PhilHealth ID, PIN and MDR: what is the difference?

PIN

Your permanent 12-digit PhilHealth number, formatted like XX-XXXXXXXXX-X. Use it for every transaction.

MDR

The Member Data Record. A printable record of your name, birthdate, member type, employer and dependents.

ID card

A card that shows your PIN. Hospitals mostly need the PIN plus a valid government ID.
A PhilHealth ID is not the same as the PhilSys National ID. They are issued by different agencies. Keep both if you have them, and never post your PIN publicly.

PhilHealth ID requirements: what to bring

You do not apply for the ID separately. It follows registration, so the papers are the ones for your application. In short:
  • A signed PMRF
  • One or more valid government IDs
  • A PSA birth certificate if your identity needs verification
  • Two ID photos if your office requests them†

How to get a PhilHealth ID number

  1. 1Register as a member. Submit the PMRF and proof of identity through an office, your employer or an official online channel. See PhilHealth online registration.
  2. 2Receive your PIN. It is sent to your email or written in your MDR.
  3. 3Print or save your MDR. The Member Portal lets registered users view and print it.
  4. 4Ask about the ID card at your Local Health Insurance Office if you want the physical card. Availability and format can vary.†

Forgot or lost your PhilHealth number? PhilHealth number inquiry options

  • ✓Log in to your account. The Member Portal shows your record if you have registered.

Correcting errors on your PhilHealth ID or MDR

  • ✓Spelling errors, wrong birthdate or sex: submit an Updating/Amendment PMRF with a PSA birth certificate.
  • ✓Change of name after marriage: attach the PSA marriage certificate.
  • ✓Wrong dependents: update the declaration and attach proof.
  • ✓Fix errors early. Mismatched records can delay hospital claims.

Using your PhilHealth ID at the hospital

How to check that your PhilHealth ID details are correct

  • Full name spelled exactly as in your PSA birth certificate
  • Correct birthdate and sex
  • Correct civil status and spouse details
  • Complete and current list of dependents
  • Correct employer or member category
Compare your printed MDR with your IDs once a year. Errors are far easier to fix before a hospital claim than during one.

PhilHealth ID scams and fixers

PhilHealth has warned the public that its ID and MDR are free and that it has not authorized agents to process them. Do not pay anyone to “expedite” your PhilHealth ID number and never share your PIN with strangers.†

PhilHealth ID for children, spouses and dependents

Dependents are listed under the member’s record. A dependent who becomes an adult member, or a person with disability, may have their own PIN. If a dependent needs a separate record, submit the PMRF and supporting documents through your office.

PhilHealth ID card: what it is and when you get one

PhilHealth has issued a standard ID card design and has said that the ID is free.† In practice, many members function with just their PIN and MDR, because hospitals verify records electronically. If you want the physical card, ask your Local Health Insurance Office whether it is available for your member type and how long it takes.
